Job Summary

Shropshire Community Health NHS Trust is seeking a skilled and dedicated Community Staff Nurse to join our South West Neighbourhood Team. As a key member of our team, you will provide high-quality nursing care to patients in their own homes or chosen place of residence, promoting timely discharge and enabling individuals to reach their full potential.

Main Responsibilities
  • Conduct holistic assessments of patients' complex and simple care needs, developing and implementing effective care plans in collaboration with the multidisciplinary team.
  • Deliver high-quality care to patients, working autonomously and delegating tasks to other team members as necessary.
  • Collaborate with other healthcare professionals to promote independence and empower patients to manage their own care.
  • Participate in the development and implementation of care pathways, ensuring seamless transitions between services.

Requirements
  • RGN NMC Registered
  • Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
  • Post-registration general experience
  • Experience of working in a multi-disciplinary/multi-agency health/social care environment
  • Experience and knowledge of basic administration systems, including Microsoft Office
  • Experience of successful partnership working arrangements with other services
  • Previous community experience
