As a key member of our team, you will provide high-quality nursing care to patients in their own homes or chosen place of residence, promoting timely discharge and enabling individuals to reach their full potential.
Main Responsibilities- Conduct holistic assessments of patients' complex and simple care needs, developing and implementing effective care plans in collaboration with the multidisciplinary team.
- Deliver high-quality care to patients, working autonomously and delegating tasks to other team members as necessary.
- Collaborate with other healthcare professionals to promote independence and empower patients to manage their own care.
- Participate in the development and implementation of care pathways, ensuring seamless transitions between services.

Requirements- RGN NMC Registered
- Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
- Post-registration general experience
- Experience of working in a multi-disciplinary/multi-agency health/social care environment
- Experience and knowledge of basic administration systems, including Microsoft Office
- Experience of successful partnership working arrangements with other services
- Previous community experience
