About the Role

We are seeking a highly motivated and experienced Cancer Care Coordinator to join our team at Dr Perkins Practice. The successful candidate will play a key role in supporting the early cancer diagnosis and cancer care quality improvement work within our Primary Care Network.

Key Responsibilities
  • Promote cancer prevention through education events, poster and leaflet distribution, and social media advertising.
  • Work with local community groups to provide patient information, awareness, and support.
  • Identify at-risk populations through the QOF register and develop systems to improve cancer screening uptake.
  • Coordinate referrals and manage safety netting processes to ensure timely diagnosis and treatment.
  • Gather and interpret data to improve coding, screening, and health inequalities.
  • Drive up standards of personalized patient care through involvement in early phases after diagnosis.
  • Support practices to deliver their quality improvement plans for early cancer diagnosis.
  • Ensure patients receive a Cancer Care review in line with national defined timescales and targets.
