Job Title: Qualified Social Worker

Working Schedule: Monday to Friday, Hybrid Working

Working Hours: 37 hours per week

Assignment Period: Ongoing (3-6 months)

Main Responsibilities:
  • A highly experienced and qualified social worker is required to support an Adult Social Care Team in a fast-paced environment.
  • Work within the Adult Access Team at Cunard Building, providing assessment and reablement support, as well as assessing eligibility for ongoing support within the community.
  • Develop and implement interventions to prevent, reduce, and delay care and support provision.
  • Manage a varied caseload of adults aged 18+ with complex needs, including safeguarding enquiries, hoarding concerns, substance misuse, mental health, learning disability, and more.
  • Utilize Liquid Logic efficiently to ensure effective case management.
  • Conduct thorough Reablement assessments and support plans, Care Act assessments, Mental Capacity Act assessments, review cases, and provide packages of care, including day centre, supported living, and respite placements.
  • Participate in a duty system to resolve unallocated cases.
  • Collaborate with partner agencies in the Liverpool area, receiving guidance and advice from a Team Manager or Senior Practitioner.
Requirements:
  • Minimum 2 years' experience as a Qualified Social Worker
  • DBS clearance
  • Valid passport for right to work requirements
