Adult Care Coordinator

We are seeking an Adult Care Coordinator to join our team at Central & North West London NHS Foundation Trust. The successful candidate will be responsible for coordinating the delivery of adult care services in a prison setting.
This is a rewarding role that requires strong organizational skills, excellent communication abilities, and a passion for providing exceptional patient care.
Duties and Responsibilities:- Coordinate the delivery of adult care services, including triaging patients, assessing their needs, and developing care plans.
- Work collaboratively with multidisciplinary teams to provide comprehensive care to patients.
- Develop and maintain effective relationships with colleagues, patients, and other stakeholders.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.
- Registered General Nurse qualification (Level 1).
- Post-registration experience in a primary care or A&E setting.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Good working knowledge of health promotion and disease prevention.
£45,000 - £65,000 per annum.
