Care and Wellbeing Coordinator Specialist

The role of a Complex Care Manager at Leaders In Care Recruitment Ltd is to provide high-quality care services to clients in the Milton Keynes area. This field-based position offers a salary of up to £35,000 per annum, plus a £3,000 car allowance.
Key Responsibilities:
- Conduct thorough assessments and implement personalized care plans for clients.
- Induct support workers and supervise team members to ensure seamless delivery of care services.
- Perform spot checks and risk management to maintain high standards of care.
- Attend meetings with Integrated Care Boards (ICBs) and clients to discuss care plans and progress.
- Provide regular reports and updates on client care and well-being.
Requirements:
- NVQ Level 3 (or equivalent) in Health and Social Care or a related field.
- Proven track record in managing and coordinating care for clients in a community setting.
- Experience in developing and implementing care plans that cater to individual client needs.
- Supportive management style that encourages team members to excel in their roles.
- Advanced communication skills, both verbal and written.
- Full UK driving license for client visits.
