Young Adults Case Manager

**Job Overview:** We are looking for a skilled Young Adults Case Manager to join our team at Corus Consultancy. As a case manager, you will be responsible for supporting young adults in achieving their goals, navigating complex systems, and overcoming challenges.
">About the Role:
In this role, you will work closely with young adults, identifying their needs, developing individualized plans, and implementing strategies to promote their well-being and independence. You will also collaborate with internal departments and external agencies to ensure seamless support services are provided.
Responsibilities:
- Conduct thorough assessments of young adults' needs and develop comprehensive plans to address these needs.
- Deliver effective interventions, including counseling, coaching, and advocacy.
- Establish and maintain positive relationships with young adults, their families, and other stakeholders.
- Stay up-to-date with best practices and industry developments through ongoing training and professional development.
Requirements:
- A degree in social work or a related field is desirable.
- Proven experience working with young adults in a residential or community setting.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a team.
Salary:
The estimated salary for this role is £28,000 - £35,000 per annum, depending on experience.
