Job Description

This complex care coordinator role involves providing day-to-day management of the rehabilitation process for multiple patients with spinal cord injuries. Key responsibilities include:

  • Supporting patients and their families through adjustments to change and loss
  • Collaborating with multidisciplinary teams regarding rehabilitation and discharge processes
  • Preparing highly complex care plans and recommending appropriate care provisions
  • Negotiating with community teams for provision of services and equipment
  • Facilitating discharges for patients with physical disabilities and complex needs

You will also be responsible for preparing specialized training for community professionals, participating in the SCIC 'lifetime' surveillance and support service, and empowering patients to make informed choices about their rehabilitation.

Person Specification

To succeed in this role, you will need to demonstrate effective leadership and interpersonal skills, as well as the ability to think creatively and work beyond your professional remit. You should possess empathic communication skills, especially when engaging with patients who have disabilities. Relevant professional qualifications, such as Occupational Therapy or Physiotherapy, are essential, along with extensive experience working with people with disabilities, ideally those with spinal cord injuries.

What We Offer

We offer a competitive salary of £45,000 - £60,000 per annum, depending on experience, as well as opportunities for career progression and professional development.
