Key Responsibilities:
This role requires a caring professional who can provide personal care to residents, including bathing, toileting, dressing, and help with feeding.
You will also administer medication, assist with daily activities, hobbies, or outings, and follow care plan guidance alongside Company policies and procedures.
As a Positive Intervention Practitioner, you will uphold high standards of Positive Behaviour Support in line with policy and practice.
You will develop and maintain a special interest in recovery-based approaches and support for people with learning disabilities and/or mental health difficulties.
You will visit and coordinate ongoing behaviour monitoring of challenging behaviours in relevant CHOICE CARE assigned service.
You will assist in the development of interventions aimed to reduce the intensity, frequency, and potential challenge of behaviours that are challenging to the individual and the service.
You will offer clearly specified individual and group work to people who receive Choice Care services under the supervision of the Clinical Lead and/or the Senior Practitioner.
You will facilitate the smooth and efficient implementation of the Care Plan systems within the homes.
