About the Role
Closer2Home is a specialist intensive home treatment service that supports young people with acute mental health problems in their homes by providing intensive input and support using a bespoke package of care tailored to each young person's needs.
We operate 365 days a year from 8am-8pm to offer flexibility to young people at times when they need it the most. The teams work in an integrated way, supporting one another to provide the service either remotely or sometimes by crossing borders into another trust's area. This means that a YP's needs are not restricted by boundaries, staff sickness has less of an impact on service delivery and YP receive a better service wherever they live.
This is an exciting opportunity for a skilled and passionate clinician to develop their career and make a powerful impact on the lives of young people when they need it most. Hospital admission can be lifesaving but often has a detrimental effect on young people as well. Closer2Home provides a service that helps young people in crisis to remain in their own home, in their own community where they can get involved in meaningful activities that are worthwhile to them.
Main Responsibilities
- Provides assessment and intensive support for young people with mental health needs who present high levels of risk
- Supports assessment prior to admission to inpatient care, where required
- Delivers planned intensive home or community-based treatment services, as well as specialist intensive home treatment for young people whose needs exceed the threshold for Tier 3 CAMHS
- Supports discharge to facilitate timely discharge from hospital
- Supports the reduction of health inequalities in children and young people's mental health pathways
- Each team has been modelled to have an optimal number and skill mix of staff to effectively address the needs of the young people the service will support.

Person Specification
Qualifications
- Nationally recognised core qualification RN, RMN, Social work, Psychology
- Professional Qualification at first degree level or equivalent
- Completion of further post-qualification specialist training at postgraduate level
Experience
- Experience of specialist mental health assessment and treatment of children and adolescents and their families/carers where appropriate as a full member of a multi-disciplinary team located in community, primary care, outpatients or inpatient settings
- Experience of working with a wide variety of children, adolescents and their families/carers across the whole range and presenting problems that reflect the full range of clinical severity
- Post qualifying experience of working as a specialist mental health worker within a mental health setting
