This role is part of the Short Breaks Team, based at Loxley House, but offers hybrid working arrangements. We have an exciting opportunity for talented individuals to join our workforce.

As a Family Engagement Coordinator, you will play a vital role in supporting families and disabled children/young people to access person centred, appropriate short breaks provision.

  1. Job Requirements:
  • You will need to process applications made directly by families/professionals and de-escalations from the Whole Life Disability Team, enabling access to Short Breaks Services for disabled children and young people in line with pathway one and two provision.
  • You will assess families needs through utilising application forms, professional supporting letters, liaison with schools and other agencies and the Resource Allocation System (RAS), including home visits and other visits necessary to gain a holistic view of the child/young person and capture their voice.
  • You will complete annual reviews for Pathway Two support packages and process Pathway One re-applications, to ensure services are still appropriate and meeting needs.
