Job Title: Field Care Supervisor

We're seeking an experienced Field Care Supervisor to join our team, responsible for supporting the delivery of exceptional care services in the community.

Key Responsibilities:

  • Support the planning and delivery of care services, working closely with Care Coordinators and care workers.
  • Conduct regular reviews of individual services, including risk assessments and needs evaluations.
  • Participate in multi-disciplinary reviews of service users' needs, ensuring seamless delivery of holistic care packages.
  • Shadow new care workers to ensure competence and provide on-site supervision and evaluation of care practice.

Requirements:

  • Experience in carrying out initial risk and needs assessments, working with service users, families, and healthcare professionals.
  • Ability to work in partnership with stakeholders to deliver high-quality care services.
  • Strong communication and interpersonal skills, with a focus on empathy and understanding.
