Job Title: Field Care Supervisor

We're seeking an experienced Field Care Supervisor to join our team, responsible for supporting the delivery of exceptional care services in the community.

Key Responsibilities:

  • Assess and review individual services to ensure the highest quality care support
  • Supervise and instruct care workers in the field, ensuring they have the necessary skills and knowledge to deliver excellent care
  • Work with Care Coordinators to plan realistic and effective care worker rotas, taking into account individual needs and skills
  • Conduct regular reviews of individual services, including risk assessments and needs analysis
  • Participate in multi-disciplinary reviews of service users' needs, ensuring a holistic approach to care

Requirements:

  • Experience in carrying out initial risk and needs assessments, either in the service user's home or other settings
  • Ability to work in partnership with service users, their family, carers, advocates, and healthcare professionals to deliver seamless care support
