Job Title: Part Time Field Care Supervisor

Job Summary:

We are seeking a highly skilled and experienced Field Care Supervisor to join our team at Constance Care Limited. As a Field Care Supervisor, you will play a crucial role in supporting the delivery of high-quality care support services to our service users.

Key Responsibilities:

  • Support the planning and delivery of care support services to service users in their homes and communities.
  • Conduct regular reviews of individual services to ensure they meet the needs of service users.
  • Supervise and support care workers in the field to ensure they are providing high-quality care.
  • Work collaboratively with care coordinators to plan realistic and effective care worker rotas.
  • Participate in multi-disciplinary reviews of service users' needs and develop care plans to meet their requirements.

Requirements:

  • Experience in carrying out initial risk and needs assessments for service users.
  • Ability to work in partnership with service users, their families, carers, advocates, and healthcare professionals.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
