Job Title: Field Care Supervisor

We're seeking an experienced Field Care Supervisor to join our team, responsible for supporting the delivery of exceptional care services in the community.

Key Responsibilities:

  • Support the planning and delivery of care services, working closely with Care Coordinators and care workers.
  • Conduct regular reviews of individual services, including risk assessments and needs analysis.
  • Provide supervision and guidance to care workers, ensuring they have the necessary skills and knowledge to deliver high-quality care.
  • Participate in multi-disciplinary reviews of service users' needs, ensuring a holistic approach to care delivery.

Requirements:

  • Experience in carrying out initial risk and needs assessments, working with service users, their families, and healthcare professionals.
  • Ability to work in partnership with stakeholders to deliver seamless care packages.
  • Strong communication and interpersonal skills, with the ability to build rapport with service users and colleagues.
