About the Role

We are seeking a highly skilled and compassionate Support Assistant to join our team at Home Care Direct. As a Support Assistant, you will play a vital role in providing exceptional care and support to our clients in their own homes.

Key Responsibilities
  • Provide personal care and support to clients with complex needs, including assistance with daily living activities, medication management, and mobility support.
  • Develop and maintain strong relationships with clients, their families, and other healthcare professionals to ensure seamless care delivery.
  • Work collaboratively with our care team to identify and address client needs, and implement care plans that promote independence and well-being.
  • Conduct thorough risk assessments and develop strategies to mitigate risks and ensure client safety.
  • Maintain accurate and detailed records of client interactions, care plans, and progress.
Requirements
  • Minimum 6 months of experience in a care-related role, preferably in a home care setting.
  • Valid Enhanced DBS disclosure and ability to work with vulnerable adults.
  • Full UK driving license and access to a reliable vehicle.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong focus on client-centered care.
  • Basic computer skills and familiarity with care management software.
