Job Summary:
We are seeking a highly skilled and compassionate Support Assistant to join our team at Home Care Direct. As a Support Assistant, you will play a vital role in providing personalized care and support to our clients in their own homes.
Key Responsibilities:
- Provide high-quality care and support to clients with complex needs, including personal care, medication management, and mobility assistance.
- Develop and maintain strong relationships with clients, their families, and other healthcare professionals to ensure seamless care coordination.
- Conduct thorough assessments of clients' needs and develop personalized care plans to meet their unique requirements.
- Collaborate with our multidisciplinary team to ensure comprehensive care and support services are delivered.
- Maintain accurate and up-to-date records of client interactions, care plans, and progress.
- Participate in ongoing training and professional development to enhance skills and knowledge in areas such as medication management, wound care, and dementia care.
Requirements:
- Minimum 1 year of experience in a care or support role, preferably in a home care setting.
- Valid driver's license and access to a reliable vehicle.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information.
- Basic computer skills and familiarity with electronic records systems.
