**Job Title:** Community Care Coordinator

We are seeking a skilled Community Care Coordinator to join our team at the NHS. The successful candidate will play a vital role in supporting individuals with acute physical illness, ensuring they receive the necessary care and resources.

The ideal candidate will possess excellent communication skills, be flexible, and able to work well under pressure. A strong understanding of community resources and experience in data collection and analysis would be beneficial.

**Salary:** £25,000 - £30,000 per annum (dependent on experience)

This is a full-time position, working 37 hours per week, including some weekends and bank holidays. We offer a range of benefits, including professional development opportunities and a supportive work environment.

Key Responsibilities:

  1. To take telephone calls from GPs, healthcare professionals, and other external agencies
  2. To update dashboards and spreadsheets with relevant data
  3. To complete and send referrals for planned resources as appropriate
  4. To provide good quality advice and information to customers and professionals
  5. To ensure all components of the customer plan are implemented as requested

Requirements:

  • GCSE Maths and English Grades 4 and above or equivalent
  • Experience in data collection and analysis using Microsoft Excel
  • Knowledge of community resources and experience in community working
  • Ability to deal sensitively with service users who may have high levels of anxiety or challenging behaviour
