Job Description:
To provide a high-quality, person-centred approach to care delivery that always considers people's safety, privacy, and dignity. This includes providing advice to others regarding the management and care of patients/service users.
Key Responsibilities:
1. Devise programmes of care (Personalised Care and Support Plans) for care home staff to deliver, monitor, and review to ensure they are delivered appropriately.
2. Support the planning, implementation, and review of health improvement programmes within the care home environment.
3. Support the proactive identification, monitoring, provision, and coordination of care for the care home residents in accordance with the Care Home Standard Operating Procedure.
