Job Title: Advanced Nurse Practitioner

We are seeking a highly skilled and motivated Advanced Nurse Practitioner to join our team at Oaklands Health Centre. As an Advanced Nurse Practitioner, you will play a vital role in providing high-quality patient care and contributing to the success of our patient-focused team.

Key Responsibilities:
  • Deliver a high standard of patient care through triage of e-consultations, telephone consultations, and face-to-face appointments.
  • Provide a minor illness service for patients registered at Oaklands Health Centre via telephone consultation or face-to-face appointments.
  • Develop, implement, and embed health promotion and well-being programs.
  • Support the clinical team in the development of individual treatment plans for chronic disease patients.
  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on their health.
Requirements:
  • Registered with the appropriate professional body (ACP/ANP).
  • MSc in Advanced Practice.
  • Experience of minor illness management, online consultation/triage, autonomous working, and working as a practice nurse or community nurse.
Skills and Abilities:
  • Excellent communication skills (written and oral).
  • Clear, polite telephone manner.
  • Competent in the use of Office and Outlook.
  • Effective time management (planning and organizing).
  • Ability to work as a team member and autonomously.
  • Good interpersonal skills.
  • Problem-solving and analytical skills.
  • Ability to follow clinical policy and procedure.
Clinical Knowledge and Skills:
  • Venepuncture.
  • Chaperone procedure.
  • Requesting pathology tests and processing results, advising patients accordingly.
  • Asthma.
  • CHD.
  • Immunisations/vaccinations (e.g., COVID and flu).
  • Understanding of evidence-based practice.
  • Broad knowledge of clinical governance.
  • Ability to record accurate clinical notes.
  • Ability to work within own scope of practice and understanding when to refer to GPs.
