Family Health Practitioner

As a Family Health Practitioner at SW Healthcare, you will be responsible for providing high-quality patient care and services in line with the General Medical Services (GMS) contract. Your duties will include conducting patient consultations, duty doctor cover, and home visits, while considering physical, psychological, and social factors when diagnosing, treating, and recommending care.
Key Responsibilities- Conduct patient consultations and home visits, adhering to the GMS contract and QOF targets.
- Collect, record, and maintain accurate and timely patient information using the practice's clinical system.
- Request, perform, and interpret tests, prescribe or administer treatments, medications, and vaccinations as necessary.
- Monitor patients' conditions and adjust treatments accordingly.
- Refer patients to specialists or other healthcare providers promptly, following established protocols.
- Promote health education and address local health and social issues.
- Complete clinical administration tasks, including audits and third-party reports.
- Liaise with healthcare professionals and support the nursing team.
- Protect patient confidentiality and adhere to practice policies and guidelines.
- Ensure effective communication with team members, patients, and other professionals.
- Participate in practice meetings and contribute to team decisions.
- Comply with Health and Safety policies and legislative requirements.
- Participate in GP revalidation, appraisals, and maintain professional development.
- Stay updated on medical developments, medications, and treatments.
To be successful in this role, you will need to possess excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment. You will also need to be able to maintain accurate and timely records, and adhere to the GMS contract and QOF targets.
