We are seeking a highly skilled and experienced Social Worker to join our team at St Andrew's Healthcare. As a Social Worker, you will play a vital role in providing holistic individualized treatment plans to our patients, working in partnership with our multi-disciplinary teams to achieve the best possible outcomes.
Main Responsibilities
- Develop and implement individualized treatment plans in collaboration with patients, families, and multidisciplinary teams.
- Conduct thorough assessments and risk assessments to identify patient needs and develop strategies to address them.
- Provide expert advice and guidance to patients, families, and staff on mental health legislation and practice.
- Lead on safeguarding investigations and provide evidence at Tribunals as required.
- Work collaboratively with other healthcare professionals to ensure seamless care and support for patients.

Person Specification
