Key Responsibilities- Conduct person-centred assessments in individuals' homes following a stay in hospital, ensuring they receive the right care, in the right place, at the right time.
- Work closely with people, their families, and other professionals to create care plans tailored to their needs and goals.
- Assist and enable individuals with activities of daily living, including bathing, dressing, meal preparation, medication management, personal care, and mobility support.
- Collaborate with healthcare professionals, social workers, and other team members to ensure a comprehensive and coordinated approach to care.
- Monitor and evaluate individuals' progress, adjusting care plans as needed to promote optimal recovery and well-being.
- Maintain accurate and thorough documentation of interactions, care plans, and progress.
- Previous experience in a healthcare or social care setting is preferred but not essential.
- Compassionate and empathetic nature, with a strong desire to make a positive impact on individuals' lives.
- Excellent communication and people skills, with the ability to work effectively within a multidisciplinary team.
- Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
- Ability to work independently, demonstrating initiative and reliability.
- Flexibility to work various shifts, including evenings, weekends, and bank holidays.
- A full driving license and access to a vehicle is preferred but not mandatory.
- Registration with the Scottish Social Services Council is required within six months of commencing in this role.
