Social Care Professional

We are seeking an experienced Social Worker or Experienced Social Worker to join our Integrated Adult Social Care team.
The successful candidate will be required to work with adults of all ages, including those with learning disabilities, physical disabilities, and mental health difficulties.
The role involves supporting individuals to stay well and independent in the community, working collaboratively with a range of health and social care professionals.
Key responsibilities include:
- Assessing and planning support for adults with complex needs
- Developing and implementing care plans to promote independence and well-being
- Working with multidisciplinary teams to deliver high-quality care services
- Providing guidance and support to colleagues and other professionals
We offer a range of benefits, including 26 days annual leave, flexible working opportunities, and a generous Local Government Pension Scheme. Relocation support of up to £10,000 may also be available for eligible candidates.
