Support Worker for Young Adults

Thrive Leaving Care is seeking a dedicated support worker to help young people transition to independent living. The ideal candidate will have a genuine interest in supporting individuals and be committed to empowering them to achieve their goals.
Key Responsibilities
- Provide practical and emotional support to young people aged 16-18
- Develop personalised support plans to identify strengths, needs, and aspirations
- Collaborate with professionals to ensure holistic support
- Advocate for the rights and needs of young people
- Support individuals in accessing education, training, and employment opportunities
Requirements
- A minimum of 1 year experience working with young people
- Knowledge and understanding of the challenges faced by young people leaving the care system
- Knowledge of local resources and services available to young people
- Commitment to promoting equality, diversity, and inclusion
