We are seeking a skilled Health and Wellbeing Support Worker to join our Primary Care Network team. As a key member of our team, you will work closely with GPs, Mental Health Practitioners, and Care Coordinators to support practice patients.
Key Responsibilities- Manage and prioritize your own caseload to support people in taking proactive steps to improve their physical and mental health conditions.
- Use coaching skills and motivational interviewing to motivate individuals, their families, and carers to identify their goals for care and activation.
- Develop and contribute to multi-disciplinary team meetings to effectively manage cases, particularly where there is complexity.
- Help and support people to access self-management education courses, peer support, or other interventions that support them in improving their health and wellbeing.
- Support the delivery of personalized care plans by coordinating input from a range of different professionals and services.
- Good level of education.
- Enrolled in or undertaking or qualified in an appropriate health coaching/motivational interviewing training course.
- A suitable and relevant qualification for working with adults and/or children.
- Experience of working in health, social care, education, third sector, or other support services with direct contact with people, children, families, and professionals.
- Experience of working with vulnerable adults and children.
- Experience of working positively with people facing complex social and emotional challenges.
- Ability to prioritize your own workload and meet deadlines.
- Understanding of mental ill health and emotional distress.
- Ability to work with a diverse range of people from different cultural and social backgrounds.
