Job Title: Community Recovery Navigator

Humankind is seeking a highly motivated and compassionate Community Recovery Navigator to join our Integrated Recovery team. As a key member of our team, you will play a vital role in supporting individuals with co-occurring mental health and substance use issues to achieve their full potential.

About the Role

As a Community Recovery Navigator, you will work closely with our clients to develop personalized recovery plans, providing tailored support and guidance to help them overcome challenges and achieve their goals. You will work collaboratively with our multidisciplinary team, including counselors, psychologists, and team managers, to ensure our clients receive comprehensive and effective support.

Key Responsibilities
  • Conduct recovery-focused assessments and develop personalized recovery plans in collaboration with clients and the multidisciplinary team.
  • Provide tailored support and guidance to clients, empowering them to make positive changes and achieve their treatment goals.
  • Work flexibly to coordinate care within the multidisciplinary team and with partner agencies, ensuring our clients receive safe and effective treatment.
Requirements

To be successful in this role, you will need:

  • A strong understanding of co-occurring mental health and substance use issues.
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients and colleagues.
  • Ability to work collaboratively as part of a multidisciplinary team.
  • Flexibility and adaptability in a fast-paced environment.
