Job Title: Community Recovery Navigator

Humankind is seeking a skilled and compassionate Community Recovery Navigator to join our Integrated Recovery team. As a key member of our team, you will play a vital role in supporting individuals with co-occurring mental health and substance use issues to achieve positive life changes.

About the Role

As a Community Recovery Navigator, you will work closely with our clients to identify their strengths and goals, and develop personalized recovery plans to help them overcome challenges and achieve their full potential. You will work collaboratively with our multidisciplinary team, including counselors, psychologists, and other support staff, to provide comprehensive support and guidance to our clients.

Key Responsibilities
  • Conduct recovery-focused assessments and develop personalized recovery plans for clients
  • Provide support and guidance to clients to help them achieve their treatment goals
  • Work collaboratively with the multidisciplinary team to ensure comprehensive support and guidance for clients
  • Coordinate care with partner agencies to ensure safe and effective treatment
Requirements

To be successful in this role, you will need to have:

  • Experience working with individuals with co-occurring mental health and substance use issues
  • Strong communication and interpersonal skills
  • Ability to work collaboratively as part of a multidisciplinary team
  • Flexibility and adaptability in a fast-paced environment
