Key Responsibilities

We are seeking a highly motivated Community Outreach Coordinator to join our dynamic team. The ideal candidate will possess excellent communication and interpersonal skills, with a passion for supporting individuals who are homeless or at risk of becoming homeless.

Responsibilities Include:
  • Develop and implement effective support plans for clients, prioritizing their unique needs and goals.
  • Build strong relationships with clients, outreach workers, and other stakeholders, fostering trust and cooperation.
  • Ensure seamless communication between teams, guaranteeing a coordinated approach to client care.
  • Collaborate with external partners to leverage resources and services, enhancing client outcomes.
  • Stay up-to-date with best practices in supporting individuals with complex needs, including drug/alcohol and mental health issues.
  • Work closely with colleagues to share knowledge and expertise, driving continuous improvement in service delivery.
  • Participate in regular supervision, appraisal, training, and meetings, staying adaptable and responsive to changing priorities.
