About the Role

We are seeking a highly skilled Physiotherapist to join our Rapid Response & Intermediate Care team in Buckinghamshire. As a key member of our team, you will be responsible for providing comprehensive specialist assessment, review, and evaluation of patient needs, as well as planning and delivering programs of care to promote health gain and maximize independence.

Main Responsibilities

  • Provide triage, comprehensive specialist assessment, review, and evaluation of patient needs
  • Plan and deliver programs of care to promote health gain and maximize independence
  • Allocate appropriate workloads to members of the Rapid Response & Intermediate Care team
  • Work with a range of professionals to ensure care is appropriate and delivered in a timely manner
  • Demonstrate professional skills and competence in leadership, care planning/co-ordination, and care and treatment delivery

Requirements

  • Registered Allied Healthcare Professional with HCPC - Physiotherapist
  • Degree/Diploma in Physiotherapy
  • Experience of recent Continuous Professional Development
  • Relevant postgraduate clinical training in Elderly, Long Term Conditions, Respiratory Disease, Cardiac Disease, Trauma, Orthopaedics, and End of Life
