We are seeking a skilled and experienced Physiotherapist to join our team at Aylesbury. As a key member of our community physiotherapy team, you will be responsible for providing high-quality care to patients in their homes and community settings.
Key Responsibilities:- Assess and manage patients with a range of conditions, including intermediate care and community physiotherapy needs.
- Develop and implement effective treatment plans to improve patient outcomes and quality of life.
- Work collaboratively with other healthcare professionals to provide holistic care and support.
- Conduct home visits and community assessments to identify patient needs and develop care plans.
- Provide education and support to patients and their families to promote independence and self-management.
- HCPC registration as a Physiotherapist.
- Valid driving license and access to a car for work purposes.
- Experience working in community physiotherapy and intermediate care settings.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
