  • Engage with adults and their carers to identify available resources that meet their needs, promoting independence and well-being.
  • Represent the service, adults, and carers views at meetings and exchange information, make decisions, and review progress, ensuring effective communication.
  • Maintain accurate records in line with the Adult Services Recording Policy, adhering to professional standards.
  • Pursue continuing professional development requirements of the professional regulator, staying up-to-date with legislative changes and best practices.

Requirements:

  • A Social Work Qualification or equivalent, recognized by the relevant health professional council/governing body (Social Care England).
  • Registration with Social Care England, demonstrating commitment to professional standards.

Estimated Salary: £32,000 - £38,000 per annum, depending on experience.
