Homecare Team Member for Service User Engagement

We are seeking an experienced Assessment and Reviewing Officer to join our Homecare Team. This exciting opportunity involves working closely with service users, ensuring their needs are met through effective assessment and review processes. As a key member of the team, you will contribute to achieving organisational goals and targets.
Job Description- Assessment Process: Develop and implement comprehensive assessment plans for Service Users, incorporating feedback from significant others where necessary.
- Care Package Management: Formulate and manage care packages that cater to individual service users' needs, aiming to promote independence and achieve agreed outcomes.
- Compliance and Targets: Ensure adherence to organisational policies and procedures, contributing to the achievement of service area targets.
- Maintenance of Records: Maintain accurate and up-to-date records of assessment and review processes, reports, and paperwork.
- Collaboration: Work collaboratively with other team members to ensure effective service delivery and support.
Essential Requirements
- Detailed knowledge of relevant policies and procedures, gained through experience in Domiciliary care settings.
- Able to work effectively in a team environment, collaborating with colleagues to achieve organisational goals.
- A full driver's licence and access to a vehicle are necessary for this role.
- Excellent communication and interpersonal skills, with the ability to build rapport with service users and colleagues alike.
- The role requires flexibility, including on-call duties on a rota basis.
