Job Summary

The successful candidate will be responsible for delivering high-quality patient care, working closely with the multidisciplinary team to assess, plan, and implement individualized care plans. This will involve coordinating the care of patients, supervising junior staff, and participating in clinical supervision and reflective practice.

Main Responsibilities

  • Communicate effectively with patients, relatives, and healthcare professionals to ensure a caring, sensitive, and empathetic approach.
  • Demonstrate excellent communication skills when dealing with difficult situations, providing clear and concise patient handovers to other team members.
  • Escalate concerns in a patient's condition to senior members of the nursing team, medical staff, and initiate referrals to other healthcare professionals.
  • Report Virtual Ward and departmental affairs to the Senior Nurse Coordinator/Practitioner or the Advanced Clinical Nurse Practitioner, and participate in discussions and ward meetings.
  • Presentation of a positive, professional image of the organization and the nursing team.
  • Participation in MDTs and Board Rounds.
  • Supervision and assessment of clinical support workers.
  • Prioritization of workload to manage the care of a group of patients.
  • Ensuring safe staffing levels on each shift when rostered in charge.
  • Undertaking digital assessments of patients to support remote monitoring and patient care.
  • Delivery of holistic patient care, recognizing individual needs of patients and their relatives.
  • Participation in clinical supervision and reflective practice, teaching, and supervision of students, nursing assistants, and other healthcare professionals.
  • Maintenance of accurate records relating to patient conditions.
