Clinical Nurse Specialist

Job Summary
The post holder will be responsible for delivering high-quality patient care, assessing patient needs, and developing individualized care plans. They will work collaboratively with the multidisciplinary team to ensure seamless care delivery and participate in clinical supervision and reflective practice.
Main Responsibilities
- Communicate effectively with patients, relatives, and healthcare professionals to ensure a caring and sensitive approach.
- Demonstrate excellent communication skills when dealing with difficult situations and provide clear patient handovers to other team members.
- Escalate concerns in patient conditions to senior nursing team members and medical staff, and initiate referrals to other healthcare professionals as needed.
- Report Virtual Ward and departmental affairs to the Senior Nurse Coordinator/Practitioner or Advanced Clinical Nurse Practitioner and participate in ward meetings.
- Prioritize workload to manage the care of a group of patients and ensure safe staffing levels on each shift.
- Undertake digital assessments of patients to support remote monitoring and patient care.
- Ensure the delivery of holistic patient care, recognizing individual patient needs and preferences.
- Participate in teaching and supervision of students, nursing assistants, and other healthcare professionals.
