Health and Wellbeing Practitioner

About the Role:

We are seeking a skilled Health and Wellbeing Practitioner to join our team. The successful candidate will be responsible for delivering client-centered treatment to individuals with substance misuse and low-level mental health needs.

Main Responsibilities:
  • Manage a caseload of clients and provide harm reduction advice and guidance.
  • Assess, care plan, and deliver structured 1:1 and group-work sessions.
  • Develop and contribute to release plans that meet the needs of service users.
Requirements:
  • Demonstrable experience of carrying out risk assessments and designing SMART care plans.
  • A thorough knowledge of the 12-step process of recovery from addiction.
  • Experience of providing counselling/CBT services to prisoners or those with drug and/or alcohol problems.
