Job Description

We are seeking an experienced Community Dementia Support Professional to join our team at Derbyshire Healthcare NHS Foundation Trust.

About the Role

This is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality care and support to individuals with dementia and their families.

You will work closely with our multidisciplinary team to deliver a range of services, including education, assessment, and support to people with dementia and their carers.

Your key responsibilities will include:

  • Providing information and support to individuals with dementia and their families
  • Working with healthcare professionals to develop and implement personalized care plans
  • Delivering education and training to carers and other stakeholders
  • Collaborating with local organizations to improve access to dementia services

Requirements

To be successful in this role, you will need:

  • A Level III NVQ in Health and Social Care or equivalent qualification
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic IT skills and ability to use electronic systems
  • Knowledge of dementia and its impact on individuals and families
