About the Role

The Community Connector Service is a vital initiative aimed at reducing social isolation and loneliness among adults in Portsmouth. As a key member of our team, you will play a crucial role in identifying and supporting individuals who are lonely or isolated, and helping them access community-based opportunities that promote health and wellbeing.

Key Responsibilities
  • Work collaboratively with the Adults Intermediate Care Team (AICT) to identify and support individuals who are lonely or isolated
  • Liaise with hospital teams, including the Accident and Emergency Department, to ensure seamless support for individuals
  • Support clients to set realistic goals and access community opportunities that promote health and wellbeing
  • Develop and maintain relationships with local community groups and organizations to promote the service and identify opportunities for collaboration
  • Conduct assessments and reviews to ensure the service is meeting the needs of individuals and the community
Requirements
  • Excellent communication and interpersonal skills
  • Ability to converse at ease with customers and provide advice in accurate spoken English
  • Knowledge and experience of safeguarding legislation
  • Extensive experience of working face-to-face in a social care or community setting
  • Ability to work flexibly, including occasional evening and weekend work
  • Access to transport or ability to easily access public transport
