About the Role
The Community Connector Service is a vital initiative at Portsmouth City Council, aimed at reducing feelings of social isolation and loneliness among adults. As a key member of our team, you will play a crucial role in identifying and accessing community-based opportunities that promote health and wellbeing.
Key Responsibilities
- Collaborate with the Adults Intermediate Care Team (AICT) based at Queen Alexandra Hospital to support clients with complex needs.
- Liaise with hospital teams, meeting individuals before discharge or in the Accident and Emergency Department to provide timely support.
- Support clients to identify realistic goals and access community opportunities within their local community.
- Develop and maintain relationships with local community groups and services to enhance client outcomes.
- Contribute to the development of the Community Connector Service, identifying opportunities for improvement and implementing changes as needed.
Requirements
- Excellent communication and interpersonal skills, with the ability to converse at ease with customers and provide advice in accurate spoken English.
- Knowledge and experience of safeguarding legislation, with a strong understanding of adult social care principles.
- Extensive experience of working face-to-face in a social care or community setting, ideally with a caseload.
- Flexibility to meet the needs of clients, including occasional evening and weekend work.
- Ability to converse at ease with customers and provide advice in accurate spoken English.
