Hospital Community Liaison

We are seeking a skilled and compassionate Community Connector to join our team at Portsmouth City Council. As a Community Connector, you will play a vital role in reducing feelings of social isolation and loneliness among adults in our community.
Key Responsibilities- Work collaboratively with the Adults Intermediate Care Team (AICT) to identify and support individuals who are lonely or isolated.
- Liaise with hospital teams to ensure seamless transitions and provide support to individuals before and after discharge.
- Support clients to identify realistic goals and access community opportunities that promote health and wellbeing.
- Develop and maintain strong relationships with community partners and stakeholders to ensure effective service delivery.
- Conduct assessments and provide advice to clients to help them access community resources and services.
- Excellent communication and interpersonal skills, with the ability to converse at ease with customers and provide advice in accurate spoken English.
- Knowledge and experience of safeguarding legislation and working face-to-face in a social care or community setting.
- Ability to work flexibly, including occasional evening and weekend work, and have access to transport or be able to easily access public transport.
- Willingness to undertake professional development and training opportunities.
