Primary Care Nurse Specialist

We are seeking a highly skilled and compassionate Registered Nurse to join our dynamic practice team. As a Primary Care Nurse Specialist, you will play a vital role in delivering high-quality patient care and promoting healthy behaviors within our diverse patient population.
Main Responsibilities- Develop and deliver specialist primary nurse-led services, including promoting healthy behaviors and maintaining disease registers in liaison with administrative staff.
- Administer child and adult immunisations and vaccinations in accordance with national and local programmes, ensuring safe storage, rotation, and disposal of vaccines and drugs.
- Undertake annual reviews of patients, providing support and encouragement to individuals, families, and groups to address issues affecting their health and social wellbeing.
- Work with people with long-term conditions to support compliance with prescribed treatments and promote health and wellbeing through general education advice on diet, smoking, chronic disease management, and exercise.
- Liaise and maintain good working relationships with all members of the practice team, adhering to the NMC code of conduct and ensuring all data protection requirements are met when gathering, recording, and storing patient data.
