Key Responsibilities
  • Assess and monitor patient conditions, identifying areas for improvement and implementing effective care plans.
  • Work collaboratively with multidisciplinary teams to ensure seamless patient care and support.
  • Develop and maintain strong relationships with patients, families, and carers to promote patient-centered care.
  • Participate in ongoing education and training to stay up-to-date with best practices and evidence-based care.
Requirements
  • Current registration with the Nursing and Midwifery Council (NMC).
  • Minimum of 6 months paid experience in a Learning Disabilities hospital environment within the last 3 years.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
