Job Title: Reablement Officer

BlueGreen Recruit Ltd is seeking a highly skilled and experienced Reablement Officer to join our team. As a Reablement Officer, you will play a crucial role in providing reablement support to service users in their own homes, ensuring that staff are meeting Essential Standards of Quality and Safety, and working in accordance with directorate policy.

Key Responsibilities:
  1. Supervise a team of Community Support Workers/Reablement Workers, providing guidance and support to ensure they are meeting the required standards.
  2. Monitor working practices and quality assurance systems, providing feedback and guidance to staff as needed.
  3. Assist in the provision of training, ensuring that staff are inducted effectively and are equipped to provide high-quality care.
  4. Liaise with the Call Centre and assist in daily and weekly programming changes for Community Support Workers/Reablement Workers.
  5. Conduct risk assessments and establish service users' needs, ensuring that care is provided in accordance with policies and procedures.
  6. Maintain records of equipment, monitoring its allocation and ensuring adequate stocks for future use.
  7. Be accessible for advice and guidance to Community Support Workers/Reablement Workers, providing feedback as appropriate.
  8. Undertake formal supervision management and performance development reviews of staff, bringing matters to the attention of the line manager as needed.
  9. Monitor ongoing progress of reablement processes, providing support and guidance to Community Support Workers/Reablement Workers.
  10. Liaise with commissioning staff regarding agreed assessed needs, goals, and outcomes to be achieved by the service user.
Person Specification:

Must haves:

  1. 4 GCSE Level C qualifications or equivalent.
  2. Diploma in Health & Social Care level 2 or equivalent.
  3. Principles of Community Care.
  4. Good knowledge of Care Planning process and Essential Standards of Quality and Safety.
  5. Good knowledge of Health and Safety issues.
  6. Good knowledge of risk assessment processes.
  7. Working as a care provider.
  8. Previous supervisory/management experience in a similar or related setting.
  9. Working as part of a team.
  10. Ability to work independently using own initiative.
  11. Good written and verbal communication skills.
  12. Supervision skills.
  13. Organisational and time management skills.
  14. IT skills.
  15. Ability to fulfil the travel requirements of the post.
  16. Commitment to providing a high-quality service.
  17. Ability to pass on knowledge in an enabling way.
  18. Ability to support and promote service user independence.

Good to have:

  1. NVQ Supervisory Level 3 or equivalent.
  2. Diploma in Health & Social Care level 3 or equivalent.
  3. Knowledge of Community Care Act.
  4. Experience of recruitment and selection.
  5. Implementation of quality assurance techniques.
  6. Dealing with attendance management issues.
  7. Ability to chair meetings.
  8. Ability to produce reports.
