As a Reablement Officer, you will play a vital role in providing reablement support to service users in their own homes, ensuring that staff are meeting Essential Standards of Quality and Safety, and working in accordance with directorate policy.

Key Responsibilities:
  • Supervise a team of Community Support Workers/Reablement Workers, providing guidance and support to ensure they are meeting the required standards.
  • Work within agreed principles and guidelines, with minimal supervision from the line manager, to ensure effective delivery of reablement services.
  • Manage direct reports, providing ongoing development and training to ensure they have the necessary skills and knowledge to perform their roles effectively.
  • Monitor working practices and quality assurance systems, providing support and guidance to Community Support Workers/Reablement Workers as needed.
  • Assist in the provision of training, ensuring that Community Support Workers/Reablement Workers are inducted effectively and are aware of their responsibilities.
  • Liaise with the Call Centre and assist in daily and weekly programming changes for Community Support Workers/Reablement Workers as required.
  • Undertake first visits to service users, conducting risk assessments and establishing their needs in accordance with policies and procedures.
  • Maintain records of equipment, monitoring its allocation and ensuring adequate stocks for future use.
  • Be accessible for advice and guidance to Community Support Workers/Reablement Workers, providing them with feedback as appropriate.
  • Undertake formal supervision management and performance development reviews of staff as required, bringing matters to the attention of the line manager as needed.
Person Specification:
  • Must have 4 GCSE Level C qualifications or equivalent.
  • Diploma in Health & Social Care level 2 or equivalent.
  • Principles of Community Care.
  • Good knowledge of Care Planning process and Essential Standards of Quality and Safety.
  • Good knowledge of Health and Safety issues.
  • Good knowledge of risk assessment processes.
  • Working as a care provider.
  • Previous supervisory/management experience in a similar or related setting.
  • Working as part of a team.
  • Ability to work independently using own initiative.
  • Good written and verbal communication skills.
  • Supervision skills.
  • Organisational and time management skills.
  • IT skills.
  • Ability to fulfill the travel requirements of the post.
  • Commitment to providing a high-quality service.
  • Ability to pass on knowledge in an enabling way.
  • Ability to support and promote service user independence.
