Job Summary

We are seeking a highly skilled and compassionate Clinical Nurse Specialist to join our team at Woking & Sam Beare Hospices. As a key member of our community team, you will provide specialist care and support to patients with life-limiting illnesses and their families.

Key Responsibilities

  • Provide specialist knowledge and expertise in caring for patients with life-limiting illnesses and disseminate such knowledge to the healthcare team.
  • Independently manage own caseload of patients and associated responsibilities.
  • Assess, plan, implement, and evaluate all aspects of patient care.
  • Provide symptom control, social, emotional, and spiritual care for patients, families, and carers.
  • Provide supportive care to patients, their families, and carers.
  • Monitor patients on an ongoing basis, evaluate outcomes, and adjust care plans to ensure optimum coordination and delivery of care.
  • Deliver high standards of care directly and assist in the management of junior staff.
  • Develop and maintain effective relationships with all members of the multi-disciplinary team to ensure collaborative working for the benefit of patient care.

Requirements

  • Registered Nurse
  • Relevant post-registration courses
  • Palliative care qualification
  • 1st level degree or masters course
  • Independent Nurse Prescriber
  • Management/leadership course
  • Mentorship/teaching qualification
