Job Title: Paediatric Community Nurse - Complex Care Specialist
Job Summary:
We are seeking an experienced Paediatric Community Nurse to join our team at Thornbury Community Services. As a Paediatric Community Nurse - Complex Care Specialist, you will be responsible for delivering high-quality care and support to young people with complex needs in a community-based setting.
Key Responsibilities:
- Providing bespoke care and support to young people with complex needs, including those with Cornelia De Lange syndrome, ASD, and Bilateral profound sensorineural hearing loss.
- Working collaboratively with the client's family and other healthcare professionals to develop and implement individualized care plans.
- Administering medication, performing tracheostomy care and suction, and managing ventilation via tracheostomy.
- Providing education and support to the client's family on care and management of the client's condition.
- Participating in multidisciplinary team meetings to discuss client care and develop strategies for improving outcomes.
Requirements:
- Minimum of 6 months' experience in the last 2 years as a Paediatric Community Nurse.
- Recent experience in tracheostomy care and suction, tracheostomy tube change, ventilation via tracheostomy, humidification, and care and use of gastrostomy.
- 12 months' minimum UK-based experience.
