We are seeking an experienced Paediatric Community Nurse to join our team at Thornbury Community Services. As a Paediatric Community Nurse - Complex Care Specialist, you will be responsible for delivering high-quality care and support to young people with complex needs, enabling them to stabilise at home and achieve their personal goals.

Key Responsibilities:

  • Deliver bespoke care and support to young people with complex needs, including those with Cornelia De Lange syndrome, ASD, and Bilateral profound sensorineural hearing loss.
  • Work collaboratively with the client's family and other healthcare professionals to develop and implement individualised care plans.
  • Provide education and training to the client and their family on managing complex care needs.
  • Contribute to the development and implementation of policies and procedures related to complex care.

Requirements:

  • Minimum of 6 months' experience in the last 2 years in a Paediatric Community Nurse role.
  • Recent experience in tracheostomy care and suction, tracheostomy tube change, ventilation via tracheostomy, humidification, care and use of gastrostomy, and changing gastrostomy buttons.
  • 12 months' minimum UK-based experience.
