Key Responsibilities- Conduct thorough assessments of patients with mental health difficulties, identifying their needs and developing personalized care plans.
- Collaborate with multidisciplinary teams to ensure seamless care pathways and effective communication.
- Provide education and support to patients and their families, promoting mental health awareness and empowerment.
- Participate in daily Red Board meetings to discuss patient progress and develop strategies for improvement.
- Maintain accurate and comprehensive documentation, adhering to NHS standards and guidelines.
- A minimum of 6 months paid experience as a Community Psychiatric Nurse in a UK hospital environment within the last 3 years.
- Compliance with NMC Professional Registration and adherence to current nursing practices.
- A full UK driving license and own vehicle (dependent on the Trust).
