About the Role
We are seeking a skilled Community Psychiatric Nurse to join our team at NHS Professionals Limited. As a Community Psychiatric Nurse, you will play a vital role in delivering mental health support and interventions directly within the community.
Key Responsibilities
- Providing robust assessments of patients who have been referred due to presenting with mental health difficulties.
- Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing.
- Signposting to appropriate care providers, including primary and tertiary care.
- Raising concerns to senior staff for support and management.
- Completing documentation and planning care for patients.
- Presentation of cases to Multi-Disciplinary Team for discussion around care pathway.
- Liaison with colleagues nursing, occupational therapy, medics, psychology, and admin.
