Job Title: Community Psychiatric Nurse

About the Role:

We are seeking a skilled and compassionate Community Psychiatric Nurse to join our team at NHS Professionals Limited. As a Community Psychiatric Nurse, you will play a vital role in delivering high-quality mental health care to patients in their own communities.

Key Responsibilities:

  • Conduct robust assessments of patients with mental health difficulties, identifying appropriate care pathways and ensuring the least restrictive option to meet their needs.
  • Signpost patients to appropriate care providers, including primary and tertiary care, and attend daily Red Board meetings.
  • Raise concerns to senior staff for support and management, and complete documentation and planning care for patients.
  • Present cases to Multi-Disciplinary Teams for discussion around care pathways, and liaise with colleagues from nursing, occupational therapy, medicine, psychology, and administration.

Requirements:

  • A minimum of 6 months paid experience as a Community Psychiatric Nurse in an UK hospital environment within the last 3 years.
  • Comply with NMC Professional Registration and keep up to date with current nursing practices.
  • A full UK driving licence and own vehicle.
