Key Responsibilities:
- Assess and deliver an agreed plan of care to individuals within a community setting.
- Work autonomously within a dynamic environment as part of the wider HSCP.
- Collaborate with the Primary Health Care team and District Nurse to provide comprehensive care.
- Develop and maintain effective relationships with patients, families, and healthcare professionals.
- Participate in continuing professional development to enhance skills and knowledge.
Requirements:
- Significant nursing experience and demonstrable leadership and communication skills.
- Passion for a career in community services and motivation to work in a dynamic environment.
- Current driving license and ability to travel to various locations.
- Ability to work 5 days over 7, including weekends and public holidays.

Working Hours:
